About Healthy Self DPC

  • What is DPC?

    DPC clinics are a type of medical practice model that function without the dependency of insurance companies.  Instead, the practice functions on monthly patient memberships, elliminating the need for huge patient volumes that result in inefficient micro visits.  This allows for smaller patient volume, longer visits, and true  medical decision making freedom.    

  • What does DPC stand for?

    In most settings, DPC stands for Direct Primary Care.  Because Direct Primary Care can be care provided by any type of provider, DPC in this practice also means Direct Physician Care. We highlight this because we value your time and care, and believe that EVERY visit should be with the doctor you want to see.  

  • What does primary care cover?


    • Minor to moderate acute injuries: lacerations requiring sutures, wound care, simple fractures, concussions. 
    • Sick care: colds, flus, strep throat, RSV, rashes, stomach bugs, sinus infections, ear infections, UTI, Sexually transmitted infections, kidney stones, etc. 
    • Chronic disease management: asthma/copd, afib, high blood pressure, diabetes, heart failure, allergies,  acid reflux, sleep apnea, mental health, ADHD, thyroid disorders, gout,  arthritis, autoimmune disorders, etc. 
    • Annual well exams, school & work physicals, sports physicals, woman exams, annual skin mapping exams. 
    • Lifestyle counselling and Preventive Screening
    • Procedures:  wart removal/freezing, mole/skintag removal,  joint injections,  abscess drainage, sebaceous cyst/lipoma removal, IUD/nexplanon removal, etc. 

    ALL OF THESE AND MORE ARE COVERED UNDER YOUR MONTHLY MEMBERSHIP. 

  • What are your preferred methods of communication?

    Text message via the Hint App is my preferred means of communication. You may also call on the telephone but because I do no large team to  answering the phone for me, we may be with another patient and there may be short delay in getting back with you.  Email responses from my personal email may take 24-48 hours for reply. 

  • Is there anything Dr. Eller does not cover under her practice?

    I take pleasure in being able to provide a large scope of care to my patients.  However, there are only a few things I DO NOT provide:


    I DO NOT prescribe benzodiazepines for anxiety or panic disorders.  They are highly addictive and cause dementia.


    I only prescribe opiods for short term use for acute injuries that I have personally evaluated.  I DO NOT prescribe opiods during the first month of a membership.  If you join my practice already taking opioids, I will need to request all records and imaging  pertaining to why you are taking them.  If not available, I may ask for imaging to be performed prior to  prescribing.  Chronic opioid use leads to hormone disruption, dementia, and other health complications. My goal will always be  to wean patients off opioids. 


    I DO NOT perform IUD and Nexplanon insertions, though I am happy to remove them.  I simply never had a training opportunity for this, and have found that I would rarely perform them in a small practice.  I also do not like causing unnecessary discomfort for my patients.  I refer my patients to an OBGYN should they want this procedure. 


    I DO NOT practice Prenatal or Obstetric care, but I will assist in finding an OB or midwife should you need one.  I will still manage all of your other needs during your pregnancy, including pain management with osteopathic manipulation.  


  • What if I need to be seen and your regular appointment days do not work?

    Please secure message me through the hint app or call.  I will be happy to find a time that works for both of us.

Membership

  • Do you schedule patients for one time consults?

    At this time I do not accept one time consults as my business model is not supported by this method. If you are interested in my model of care please sign up for the recurring membership. This allows me to keep the number of patients I care for comparatively small so that I am highly available to them.

    If you are unsure about joining and would like to meet Dr. Eller first, please contact us and schedule a Meet and Greet appointment. 

  • I don't see a membership that fits my situation.

    Contact us! Chances are we can make something work. 

  • Why do you have an enrollment fee?

    Getting each patient onboarded in our system and having a comprehensive visit (60-90 minutes) when establishing care takes extra time and energy. We consider this an intitial investment in your health. 

  • What if I need to cancel my membership?

    We understand there are many reasons for the need to cancel your membership. We only require a 30 day notice prior to your next bill. 


    To allow your doctor to have time to get to know you and address your health care needs we require a 6 month minimum of membership. Terminations prior to the 6 month minimum will incur a $250 early cancellation fee.  Re-enrollement  into the practice is subject to the physician's discretion,   member capacity, and a $200 re-enrollment fee. 

  • How can Healthy Self DPC help small businesses?

    With the combination of DPC and a self-funded insurance plan, businesses can save tremendous amounts of money. All the while, your employees will be receiving amazing care which can help reduce sick days. Your employees will enjoy great care accessibility, not having  to waste half to a full day waiting to be seen; we can get them in with virtually no wait time.


    With your  employees getting the majority of their healthcare needs taken care of quickly and without filing an insurance claim, this leaves a large amount of the claim pool from the self-funded plan leftover at the end of the year. A percentage of the money saved can now go back to the business to use somewhere else that is needed. 

    Using this model, businesses nationwide have been paid 25-50% of their claim pool back, on top of saving money on a monthly basis.  

  • Do you offer any discounts?

    Yes, we offer a 10% discount on all memberships that are paid in full for year. There are no refunds for paying in full for the year.

     

  • How does sign up work and what happens next?

    Upon enrollment  I will send a text message to your smart phone with a link to the secure App that I use called hint. You will then be able to create a secure connection which will allow us to communicate about your health information via text or video conference. An intake form will also be automatically sent to you which will help me have a chart prepared for our first appointment. Appointments are handled through this App via text message to agree on a time that works for both of us and of course you can also call to schedule your appointment if you prefer however as I do not have anyone answering calls for me there may be a delay as I may be with a patient.

Insurance

  • Can I join this practice and still have insurance?

    Yes, whether or not you have insurance, you can be a member of this practice.  

  • I have insurance, why should I join Healthy Self DPC?

  • Can I use my insurance to pay for services?

    You cannot use your insurance to pay for your monthly membership.  However, your insurance can be used to pay for covered labs, imaging, and specialists when needed.  Their payment is not gauranteed, and if not covered you will be responsible for the charges.  You may choose to pay our wholesale prices rather than risk insurance not covering your labs or imaging. 

  • Can I use my Health savings account (HSA) as payment for my monthly membership?

    Unfortunately, no.  

    The DPC monthly fee is currently deemed as a "health plan" by the IRS, in the same way  a traditional monthly health insurance premium is called a "health plan."  The IRS  categorizes the DPC monthly membership fee as a second "health plan" and it does NOT qualify as a HSA expense.  


    However, invoices for charges such as,  lab work and OMT, can be created seperately, so that you may use your HSA for payment for non monthly memberhsip items such as these. 

  • Do you accept Medicaid or Medicare?

    No, we do not.  We believe in providing care that it is not confined to the limits of these programs. 

  • What if I need surgery or endoscopy procedure and I don't have insurance?

    We can help coordinate to find specialist in the region to find good cash pricing and financing options.  They do exist! 


  • What alternatives are available to insurance for emergencies?

    There are more affordable options for emergency coverage.  Health shares are similar to insurance but usually cost much less per month, and are a great alternative to consider.  It's always good to be prepared for the unexpected.  Why fork out money to insurance companies and never use them?  Keep your money in your pocket  with lower premiums than traditional health insurance. 


    If you are interested in learning more, we recommend you go to the websites of health share plans to get more information. Some companies include Zion Health Share, Sedera, Medishare, and Samaritan Ministries.  Many offer discounts for being a DPC member!  


Osteopathic Manipulation Medicine

    Osteopathic medicine is a distinct approach to healthcare practiced by Doctors of Osteopathic Medicine (DO). DOs account for approximately 11% of all physicians in the United States. 


    The osteopathic philosophy of care is centered on 4 Osteopathic Tenets: 

    1. The body is a unit; the person is a unit of body, mind, and spirit.
    2. The body is capable of self-regulation, healing and health maintenance.
    3. Structure and function are reciprocally interrelated.
    4. Rational treatment is based upon an understanding of the basic principles of body unity, self-regulation and the interrelationship of structure and function. 

    Osteopathic Manipulation Medicine (OMM) is practice of osteopathic philosophy and principles with hands-on care performed by your osteopathic physician. It involves using the hands to diagnose, treat and prevent illness or injury.


    DOs can be found practicing in every specialty, including primary care,  ER, surgery, OBGYN, and psychiatry.   DOs combine the  latest science and technology, while considering  options that complement medications and surgery.




  • What is Osteopathic Manipulation Treatment (OMT)?

    Osteopathic Manipulative Treatment (OMT) is a whole system evaluation & treatment method with a goal to maintain health and prevent illness or injury by restoring normal body function. 

    Using a hands-on approach, the osteopathic physician will move tissue, muscles, and joints using techniques that include stretching, thrusting, and gentle or firm pressure

  • What's a DO and how are they different from MDs?

    • A DO, short for Osteopathic Doctor, receives a medical degree from a U.S. osteopathic school. From day one of medical school, DOs are trained to look beyond your symptoms and to understand how  your environmental  and lifestyle impact your health. 
    • ∙They have a more holistic approach, considering a patient’s environment, nutrition, and body system as a whole when diagnosing and treating medical conditions.
    •  DOs are required to complete an additional 200 hours of training in osteopathic manipulative medicine, in addition to a standard education in medicine that all medical doctors must obtain.
    •  By combining this knowledge with the latest  advances in medicine, they offer patients one of  the most comprehensive approaches available in healthcare.


  • What can OMT treat?

    Goals of treatment are to relieve pain, promote healing, and improve mobility & circulation. Several conditions can benefit, including:


    Chronic pain

    Back pain in pregnancy

  • Who can benefit from OMT?

    • All ages & backgrounds
    • Can complement or replace drugs & surgery
    • Treatment will vary depending on patients
    •  age, clinical condition, and comfort level.

  • What is an osteopathic exam like?

    A physical exam begins with assessing posture, spine, and balance. The DO will use their hands to palpate the back and extremities to assess tissue changes and joint mobility.  Based on exam findings, the DO will form a unique treatment plan tailored to the patient. 


  • How are DOs different from a chiropractor?

    • DOs focus not only on the spine and muscles, but all bones and tissues that make up the body.
    • Chiropractors cannot prescribe medicine.
    • Your DO will only suggest treatment as needed, not multiple days a week.
    • DOs can adjust your spine similar to a chiropractor, they also have other techniques that don’t involve “cracking and popping”. 
  • How many OMT visits do I need?

    Each individual treatment plan is unique. Visit frequency and number of visits required will vary depending on patient’s health status, their own perception of their health, chronic conditions, environmental factors, and patient motivation. Treatment duration can vary from one to several months.

  • Can I call anytime?

    I ask that you try to respect our time after hours and please only text or call if ugrent.   


    For non urgent things, such as general questions, refill requests, appointements, etc, please message via Hint portal. 


    If having an emergency  or an urgent situation that cannot wait til the next business day please text or call via the members only phone number. 


    If after 9pm and urgent/emergent situation, please call via member only number. You may need to call twice during sleeping hours.
